Statistical Prediction of the Outcome in Severe Epidural Hematoma

Abstract

|
The relationship of outcome to the appearance of several factors as seen on initial clinical manifestation was assessed in 268 consecutive severely head-injured patients (age¡Ã20, GCS¡Â8) with acute epidural hematoma. Age, GCS score, pupillary
light
reflex, motor response to stimuli and oblitrated degree of basal cistern on CT scan at time of operation were summarized, and the probability index of all survival and mortality was cculated in each factor. Outcome could be directly related to
the
level
of consciousness, severity of impaired pupillary light reflex, absence or presence of abnormal motor response to stimuli, and oblitrated degree of basal cistern on CT scan (p<0.001). There was not statistically significant differences among each
age
group, but the outcome was poorer in the older age group.
The mortality was 45.1% and survival rate 54.9% among all factors. The probability index may be very useful for early prediction of the combination outcome in severe head-injured group associated with variable clinical manifestations.
